Title: Spare wheel

If I change my car from 17's to run on 18's do I need to change the spare wheel to an 18" spacesaver as well?

Not if you keep the rolling circumference the same.  I changed my wheels from the standard 17" Classix on my GT Sport, to 18" Zolders and now 18" Omanyts and haven't changed the space saver... never had need to use it mind you!
